Summary:Aim: to conduct the comparative assessment of intraoperative effectiveness and safety of pulmonary veins (PV) isolation by radiofrequency catheter with pressure sensor “catheter-tissue” and cryoballoon of the second generation in patients with persisting form of atrial fibrillation. Materials and methods: 70 patients with persisting atrial fibrillation (AF), resistant to antiarrhythmic therapy were included in prospective, randomized, single-center study. Patients were divided into groups randomly by cryoballoon (CB) (n=35) and radiofrequency (RF) (n=35) isolation of PV. RF isolation was conducted by radiofrequency catheter with pressure sensor “catheter-tissue” (Thermocool SmartTouch, Biosense Webster, USA) in conditions Carto 3 navigation system, cryoballoon ablation – the second generation balloon (Arctic Front Advance, Medtronic, USA). Results: In RF isolation group acute isolation was achieved in 100% of PV in CB isolation group – 97,8% of PV. Average duration of procedure was shorter in CB group, than in RF group (93,4±7,3 against 125,3±6,8 min, P<0,001). Average time of roentgenoscopy was significantly longer in CB group than in RF group (13,57±0,87 against 5,06±0,45, р<0,001). Safety profile was equal in both techniques. Conclusion: intraoperative effectiveness and safety of radiofrequency isolation of PV by catheter with pressure sensor “catheter-tissue” and cryoballoon isolation of PV by the second-generation balloon are equal. The common duration of procedure and duration of used technique in left atrium is significantly shorter during cryoballoon ablation than during radiofrequency isolation.
