| Summary: | Tämän kehittämistyön tarkoituksena on osana AVOT-hanketta kehittää Pohjois-Karjalan ikäihmisten palveluita ensihoidon näkökulmasta. Kehittämistyön tavoitteena on kotihoidolle hoidon tarpeen arviointi palvelun pyytämiseen ja ensihoidolle palvelun toteuttamiseen luotujen toimintaohjeiden kehittäminen. Toimintaohjeet tulevat Pohjois-Karjalan kotihoidon ja ensihoidon käytettäviksi. Koko Suomeen vaikuttavan sote- ja maakuntauudistuksen myötä kehitetään sosiaali- ja terveydenhuollon rakennetta, palveluita ja rahoitusta. Pohjois-Karjalassa perustettiin Siun sote – Pohjois-Karjalan sosiaali- ja terveyspalveluiden kuntayhtymä, tarkoituksena ennakoida tulevaa valtakunnallista sote-uudistusta. Siun sote toteuttaa alueensa ensihoitopalvelun yhteistyössä Pohjois-Karjalan pelastuslaitoksen kanssa. Hallituksen kärkihanke kehittää ikäihmisten kotihoitoa ja vahvistaa kaikenikäisten omaishoitoa (I&O), tarkoituksena yhdenvertaistaa ja koordinoida palveluita. Pohjois-Karjalassa Siun sote toteuttaa I&O-kärkihankkeen yhtä kahdeksasta kokeiluhankkeesta, AVOT-hanketta, joka tarkoittaa arvokasta vanhenemista omatoimisuutta tukemalla. AVOT-hankkeen toimiva kotihoito kehittämiskokonaisuuden yksi osa-alueista on akuuttitilanteiden toimintamalli. Toimintamallina on ensihoidon suorittama hoidon tarpeen arviointi kotihoidon asiakkaille kiireettömissä epäselvissä tilanteissa. Toimintamalli otetaan aluksi Kiteellä kokeiluun pilot-jaksolle 15.5.–31.10.2017 välisenä aikana. Toimintaohjeet luotiin hoidon tarpeen arviointi pilot-jaksolle, jonka aikana ne testattiin. Pilot-jakson loputtua luotuja toimintaohjeita kehitettiin Kiteen kotihoidolle ja ensihoidolle tehdyn Webropol-kyselyn vastauksien ja luodun teoreettisen viitekehyksen perusteella. Toimintaohjeisiin tehtiin tulosten perusteella muutamia korjauksia. Ohjeiden sisältöä muutettiin myös niin, että niitä voidaan käyttää koko Pohjois-Karjalan alueella. Pohjana käytettiin Pohjois-Karjalan pelastuslaitoksen valmista ohjepohjaa. The purpose of the thesis was to develop the service for elderly people from North Karelia from the point of view of emergency care as a part of the AVOT project. The aim is to improve created guidelines for home care to request the novel service of treatment need assessment. Another aim is to improve created guidelines for emergency care to execute the service. With the upcoming Health, social services and regional government reform of Finland, the structure, services and funding of social and health care will be developed. In North Karelia, Siun sote – Joint Municipal Authority of Social and Health Services – was established in order to anticipate the nationwide reform. Siun sote organises emergency care service in its area in cooperation with the North Karelia Rescue Department. The government’s top project, called I&O, aims to develop home care for elderly people and caregiving for all ages. It’s intended to equalise and coordinate social and health services. Siun sote implements one of the eight pilot projects of the I&O project, the AVOT project, which aims at supporting independency of the elderly. One of the sectors of the AVOT project is functional home care, which includes an untraditional emergency care unit. The unit is used to assess treatment needs of the home care clients in non-urgent and obscure situations. This operating model was tested in Kitee during the pilot period from 15th of May to 31st of October in 2017. The guidelines were created for the pilot period when they were tested. At the end of the pilot period, the guidelines were developed based on the answers of the Webropol survey, which was sent to the home care employees and paramedics of Kitee, and the theoretical framework. Based on the results of the survey, some corrections were made to the guidelines. The content of the guidelines was also modified so that they could be used throughout North Karelia.
