This work was carried out during the tenure of a Medical Research Council Scholarship for Training in Research Methods between October 1964 and September 1966, and forms part of a thesis submitted for the Ph.D. Degree of the University of London. Grateful acknowledgement is due to Dr J. G. Ross and Dr G.
