Summary:Introduction: Acute Respiratory Infection (ARI) is one of the most relevant diseases worldwide because it generates the highest mortality rate in children under 5 years of age. In Colombia, along 2018, there were 2´271,307 cases of ARI in children under 5 years, 9.5% were pneumonias and 12.2% bronchiolitis. However, there is a gap in knowledge about the costs related to this disease for the health care system and for the society. Objective: To estimate the cost-illness of pneumonia and bronchiolitis in children under 5 years of age in Colombia. Methodology: A partial economic evaluation to estimate the direct medical costs of the disease was done, based on information obtained from the databases of the Individual Registry of Services Provision and Sufficiency Database, as well as a calculation of the direct and indirect non-medical costs through a survey constructed, validated and applied to parents, mothers and caregivers of children hospitalized by these events. A microcosteo and Bootstraping was developed with 50,000 iterations for each event with the generation of secondary analyzes by sex, age and stay in the Intensive Care Unit (ICU). Results: The costs of illness in children under 5 years of age hospitalized for bronchiolitis or pneumonia in Colombia was I$ 2,447 (COP 3,208,414 - IC 95% COP 2,158,442 - 4,146,951) and I$ 2.104,2 (COP 2,757,963 -IC 95% COP 2,305,559 -3,603,664) respectively; showing that bootstrapping intervals overlap, and no statistically significant differences were observed between the costs of the events analyzed. Conclusions: With the results obtained, with more than 30,000 children a year hospitalized for these causes in Colombia, the costs in 2018 for the health care system was USD 30’778.479 (COP 90.994.417.999), a situation, that in the worst case scenario, hospitalization of 100% of the children, could compromise up to 2.6% of the national health budget.
