Miehiä kuolee sepelvaltimotautiin ennen 65 vuoden ikää kolme kertaa enemmän kuin naisia. Ennen vaihdevuosia oleva estrogeenituotanto suojaa naista, mutta vanhemmiten sukupuolierot tasoittuvat. Sydäntautipotilaan kuntoutus ja elämäntapamuutosten tekeminen vaikuttavat merkittävästi potilaan arkeen hänen loppuelämänsä ajan. Potilas itse voi vaikuttaa siihen, kuinka täysipainoista elämää hän pystyy elämään. Tämä opinnäytetyö toteutettiin työelämälähtöisenä projektina yhteistyössä Haaparannan terveyskeskuksen fysioterapeuttien kanssa. Projektin tarkoituksena oli tuottaa konkreettinen ja selkeä suomenkielinen informaatiolehtinen Haaparannan terveyskeskuksen fyysiseen sydänkuntoutusryhmään tuleville uusille potilaille. Projektin tavoitteena oli informaatiolehtisen avulla antaa potilaille tietoa ja helpottaa heitä muistamaan fyysisen ohjatun sydänkuntoutuksen yhteyteen liittyviä olennaisia asioista sekä näin auttaa heitä keskittymään kuntoutumiseensa. Projektin tuloksena syntyi Haaparannan terveyskeskuksen sydänkuntoutujille informaatiolehtisen muodossa tehty tietopaketti, johon oli onnistuttu keräämään etukäteen suunnitellut asiat. Sydänkuntoutusryhmän tauolle laittamisen vuoksi informaatiolehtistä ei toistaiseksi kuitenkaan voitu viedä käytäntöön, vaan se tallenneltiin sähköisessä muodossa kuntoutusryhmän fysioterapeuttien tiedostoihin, josta se tulevaisuudessa oli helppo päivittää uusiin haasteisiin. Yhdeksi tärkeäksi jatkotutkimushaasteeksi nousi sydäntautipotilaan kuntoutumiseen vaikuttava vertaistuen merkitys. Mielestäni sen tarpeellisuus nousi esille miettiessäni potilaiden halua ohjatun fyysisen kuntoutuksen lopettamisen jälkeiseen seurantaan. Coronary heart disease is a significant national disease in the western world. This dis-ease is more common among the older people but half of the patients are under 65 years of age. Three times more men die of coronary artery disease than women. The women have a better protection against this disease due to the production the estrogen before the menopause. Differences between the sexes moderate in later life. Heart disease patient rehabilitation and lifestyle changes make a significant impact on the patient's everyday life during the rest of his/her life. The patient him/herself can af-fect how full life, he/she will be able to live. This thesis was implemented as a work related project in collaboration with the Hapa-randa health care center physiotherapists. The purpose for this project was to produce a concrete and clear information leaflet in Finnish for the future, new patients of Hapa-randa health care center physical rehabilitation cardiac healthcare group. The goal of the project was, by providing this leaflet, to give information to the patients and help them remember the essential issues related to guided physical cardiac rehabilitation and thus help them to focus on their rehabilitation. As a result of this project Haparanda health care center`s cardiac rehabilitation patients got an information leaflet, which is containing all pre-designed issues. Because the car-diac rehabilitation group operation is currently on hold, it wasn't possible to introduce the leaflet at this time, but it has been saved electronically in the physiotherapist's reha-bilitation group files, which can be easily updated for the future challenges. One of the important future research topics could be the significance of peer support in the rehabilitation of heart disease patients. In my opinion, the need for it came up, while thinking the patients' willingness to the follow up process after his/her rehabilitation has been completed.
