Kotisaattohoitokansion suunnittelu ja toteuttaminen Lapin sairaanhoitopiirin ensihoidolle

Tämä opinnäytetyö käsittelee ensihoidon osallistumista kotisaattohoitopotilaan hoitoketjuun. Opinnäytetyön tarkoituksena oli tuottaa kotisaattohoitopotilaan kotiin sijoitettava kotikansio, josta kaikki potilaan hoitoketjuun osallistuvat tahot saavat helposti tiedot potilaasta. Kotikansio perustuu Pohjois-Karjalan ja Lapin sairaanhoitopiirin saattohoitomalleihin sekä asiantuntijoiden sähköpostihaastatteluihin. Muuta lähdeaineistoa saatiin järjestelmällisellä Google Scholar -tietokantahaulla. Kansio tuotettiin pdf-tiedostoksi, ja se sisältää potilastiedot sekä hoitolinjaukset, hoitosuunnitelman ja hoitovastuun jakautumisen. Valmiista kansioista kerättiin asiantuntijapalaute, jonka perusteella sitä parannettiin. Suomessa kotisaattohoitoketjun toimintaa on kehitetty uudenlaisella toimintamallilla, jossa ensihoidolla on ennakkotiedot alueella hoidettavista saattohoitopotilaista. Potilaan voinnin heiketessä ensihoito voidaan hälyttää paikalle ottamalla yhteyttä suoraan ensihoitoon, jos hoitovastuussa oleva taho ei pääse paikalle. Tutkimusten perusteella kotisaattohoitopotilaan hoidon ennakkosuunnittelu ja hyvä kommunikaatio terveydenhuollon palvelujen välillä ovat tärkeitä tekijöitä hyvän saattohoidon takaajana. Opinnäytetyössä tuotetussa kotikansiossa ensihoidolle on suunniteltu kansioon oma ensihoidon lehti, josta kaikki hoitotahot voivat nähdä ensihoidon toiminnan potilaan luona. Pyytämämme palautteen perusteella kotikansiolle on tarvetta ja se voi toimia käytännössä ensihoidon tukena. Jatkotutkimuksen aiheena voisi olla kotikansion kokeileminen käytännössä. This study focuses on emergency care combined with palliative care carried out at home. The purpose of the study was to design and implement a folder with all necessary information regarding the palliative care patient being cared for at home. The aim of the folder was that all caregivers including paramedics, could find the essential information regarding the palliative care patient easily and quickly. The folder includes advance care planning, medical treatment plan and distribution of care responsibilities. The most important directives are the decision of terminal care, the decision of DNR and the living will of the patient. The folder was made as a pdf-file and its content was based on the model of terminal care in North Karelia and Lapland Hospital Districts and on the interviews of chosen experts in the field. Studies show that the sufficient planning of terminal care at home and good communication between health care services are important in guaranteeing good terminal care. In Finland, a new model has been developed where the emergency care has the advance information regarding the palliative care patients being cared for at home in the area. In case the patient’s condition gets worse, the emergency care in the area can be alarmed directly if the main caregiver responsible for the patient is being inhibited. In the model, the emergency care has care responsibility of the palliative care patient at home. In the folder, the emergency care has its own section where all parts responsible of the patients care can see the actions of the emergency care at the patients home. According to the feedback given on the folder, there is a need for a folder like this and it might be useful in practice supporting the acts of the emergency care. Further studies of the subject could test the suitability of the folder in practice.
