REPLANTAATIOPOTILAAN HOITO-OHJE ENSIHOIDOSSA

Tiivistelmä Replantaatiolla tarkoitetaan irronneen raajan tai kehon muun osan liittämistä uudelleen kehon toimivaksi osaksi. Suomessa replantaatiokirurgiaa vaativia vammoja syntyy etenkin maataloudessa, sirkkelitapaturmissa ja teollisuuden tapaturmissa. Tavallisimmin tapaturmat kohdistuvat käsiin. Replantaatiota vaativia vammoja yläraajoissa on teollisuusmaissa arvioitu olevan 1-10 tapausta 100 000 asukasta kohden vuodessa. Opinnäytetyön tavoitteena oli yhdenmukaistaa ja selkeyttää replantaatiopotilaan hoitoa Pohjois-Karjalan pelastuslaitoksen ensihoitajille, jotka toimivat sairaalan ulkopuolisessa ensihoidossa. Tarkoituksena oli syventää ensihoitajien osaamista replantaatiopotilaan hoidossa. Opinnäytetyön tehtävä oli tuottaa selkeä ohje, jossa käsitellään replantaatiopotilaan hoito, irronneen raajan käsittely, välineet ja miten niitä käytetään. Opinnäytetyön toimeksiantaja oli Pohjois-Karjalan pelastuslaitoksen ensihoitotoimi. Ohje sijoitetaan kaikkiin ensihoitoyksiköihin, jotka toimivat Pohjois-Karjalan maakunnan alueella. Uusi ohje korvaa aikaisemman ohjeen. Opinnäytetyön tuloksena potilasturvallisuus paranee ja yhteistyö eri sidosryhmien välillä tehostuu. Uutta ohjetta päästään tulevaisuudessa testaamaan pelastuslaitoksen toiminnassa. Tässä vaiheessa sen toimivuudesta ei ole vielä lisätietoa saatavana. On huomioitava, että replantaatiota vaativia vammoja syntyy keskimäärin alle viisi vuodessa eli ne ovat harvinaisia. Jatkotutkimuksessa voisi selvittää millaisia kaupallisia replantaatiopakkauksia on olemassa ja olisivatko ne parempi vaihtoehto nykyisille replantaatiopusseille. Abstract Replantation means re-attaching a limb or another body part that has broken off so that it becomes a functioning part of the body again. In Finland, injuries requiring replantation surgery mainly occur in farming and industry accidents and in the use of circular saws. Mainly hands are injured. It is estimated that injuries requiring replantation of an upper limb occur 1-10 times a year per 100 000 inhabitants in industrialised countries. The purpose of this thesis was to unify and clarify replantation patient care administered by the paramedics of the North Karelia Rescue Department working in emergency medical care outside hospitals. The goal was to deepen the skills of paramedics in replantation patient care. The commission was to produce a clear set of instructions for the replantation patient care, the treatment of the limb that has broken off, the instruments available, and how they are used. The thesis was commissioned by the emergency care services of the North Karelia Rescue Department. The new instruction sheet will be placed in every emergency care unit in North Karelia. One outcome of the thesis will be that patient safety improves and cooperation among the different interest groups strengthens. The future will provide opportunities to test the instructions in the work of the Rescue Department. At present there is no further information available on their functionality. It is to be noted that such injuries are rare and occur less than five times a year. An interesting subject for further study would be to examine what sorts of commercial replantation packages are available and whether they would be a better alternative to the current ones.
