The aim of study - to investigate and compare the dynamics of the stroke incidence and mortality in the Irkutsk and Sakhalin regions. Material and methods. The stroke study has been carried out in the Sverdlovsk district of Irkutsk and Yuzhno-Sakhalinsk for 2009-2014 using the population-based register method, according to the protocol of the international training course MONICA (1985-1995). In the regions studied, data collection was carried out in a uniform manner by unvarying researchers team during the 6-year period. The diagnosis of stroke met the criteria of ICD 10 revision. The diagnosis was established on the basis of CT/MRI and/or autopsy in the event of a lethal outcome. All cases of acute stroke (the first 28 days from the disease onset) that occurred in the studied populations in the age group 25-74 years were recorded and analyzed. The analysis involved 2196 cases of stroke in Irkutsk and 2619 cases of stroke in Yuzhno-Sakhalinsk. Results. In 2009 the incidence of stroke among men was 4.43 cases per 1000 in Yuzhno-Sakhalinsk and 3.97 cases in Irkutsk. During 2009 to 2014, the incidence rate among men decreased by 17 % in Irkutsk ( р > 0.05) and by 23 % in Yuzhno-Sakhalinsk. The incidence of stroke among women in Irkutsk dropped by 16 % ( р > 0.05), in Yuzhno-Sakhalinsk - by 11 % ( p > 0.05). The mortality rate from stroke among men was 1.11/1000 in Irkutsk, 1.68/1000 in Yuzhno-Sakhalinsk in 2009, the indicator showed a similar decrease of 47 and 53 % in 2014 ( p < 0.01). The death rate also decreased among women in Irkutsk by 58 % ( р < 0.01) and increased by 26 % ( р < 0.05 among women in Yuzhno-Sakhalinsk).
