(accepted by the International Journal of Integrated Care - IJIC - 2023-10-31 08:05 AM GMT). This matrix is extracted from the study titled " Health System Enablers and Barriers to Continuity of Care for First Nations Peoples Living with Chronic Disease ," which responds to this question: What are the health system enablers and barriers to continuity of care for First Nations Peoples living with chronic conditions?
